Acceptance of Responsibility
I, the undersigned, understand and agree to the following terms and conditions for boarding my dog at Galway Pet Services:
Health and Behavior: I certify that my dog is in good health and has not shown any signs of aggressive behavior. I have provided accurate and complete information regarding my dog’s health and behavior.
Vaccinations: I confirm that my dog is up-to-date on all required vaccinations and I have provided proof of vaccination.
Social Environment: I acknowledge that Galway Pet Services operates a cage-free environment where dogs interact and play together. I understand that this involves inherent risks, and I accept these risks on behalf of my dog.
Liability: I accept full responsibility for any injury or damage caused by my dog while in the care of Galway Pet Services. I agree to hold harmless and indemnify Galway Pet Services, its owners, and staff from any liability arising from my dog's actions.
Emergency Care: In the event of a medical emergency, I authorise Galway Pet Services to seek veterinary care for my dog. I agree to be responsible for any veterinary costs incurred.
Pick-Up and Drop-Off: I agree to adhere to the established pick-up and drop-off times. I understand that failure to comply may result in additional fees.
Payment: I agree to pay all boarding fees and any additional costs incurred during my dog’s stay at Galway Pet Services.
